TivaCare Anesthesia — Privacy Policy

1. INFORMATION WE COLLECT

TivaCare Anesthesia, LLC ("TivaCare") collects information you provide when scheduling anesthesia appointments, including your name, date of birth, phone number, and email address. We also collect payment information processed securely through Stripe.


2. HOW WE USE YOUR INFORMATION

We use your information to schedule and confirm anesthesia appointments, process deposit and balance payments, send appointment reminders and post-procedure follow-up messages via SMS, and maintain accurate billing records.


3. SMS COMMUNICATIONS

At a TivaCare-affiliated dental or surgical office, front-desk staff read the SMS consent disclosure aloud to the patient (or parent/guardian for pediatric cases) during booking. Staff then record the patient's verbal agreement by checking an on-screen attestation checkbox; the booking will not submit until the checkbox is checked. The consent record is stored in TivaCare's booking system with a timestamp, case number, attesting staff identity, and the patient's phone number. By providing your phone number through this process, you consent to receive SMS messages from TivaCare Anesthesia at the phone number provided, including appointment confirmations, deposit and balance payment links, pre-procedure fasting and arrival reminders, and post-procedure follow-up notifications. Message frequency varies, typically 3-5 per appointment. Message and data rates may apply. Reply STOP at any time to opt out. Reply HELP for help. Consent to receive SMS is not a condition of receiving anesthesia services. Mobile information collected for SMS consent or opt-in will not be shared with third parties or affiliates for marketing or promotional purposes.


4. PAYMENT INFORMATION

Payment card information is processed by Stripe, Inc. and is never stored on TivaCare systems. Stripe's privacy policy is available at stripe.com/privacy.


5. INFORMATION SHARING

We do not sell, trade, or rent your personal information. We share information only with your dental provider for coordination of care, and with payment processors and SMS service providers as necessary to deliver services. Mobile information collected as part of SMS consent or opt-in is excluded from all sharing; this information will not be shared with any third parties or affiliates for marketing or promotional purposes.


6. DATA SECURITY

We use industry-standard security measures to protect your information. All payment transactions are encrypted using SSL technology.
